Distinguished Budget Presentation Awards Program - Application Form
To apply for GFOA's Distinguished Budget Presentation Award Program, please complete the application below. Applications must be submitted within ninety (90) days of legal adoption of the final operating budget or submission of the proposed operating budget to the governing body, unless an extension has been granted. To complete this form online, applicants will need to have the following information available.
- Electronic versions of the budget document, detailed location criteria guide, and your response to comment's from your last submission (if applicable - first time applicants will not include a response to comments)
- GFOA log-in and password. If you do not log-in with your membership information, non-member rates will apply. DO NOT create a new user account if you do not have login information. A new user account does not automatically update within our system and the member rate for your submission will not be accessible. Send an email to firstname.lastname@example.org if you require your username and password reset.
- If you are mailing a check for the fee you DO NOT need to log into the website. Just select check on the application and submit with the required documents.
As part of the process to complete this application, you will provide payment through GFOA's e-store for the application. Click on the appropriate fee to be transfered to GFOA's e-store for payment. If you do not wish to pay online and would rather pay by check, please check the box below.
After completing payment, please complete the remainder of this form and click submit. An email confirmation will be sent to the submitter.
